If you’ve noticed that the title of a submitted form response doesn’t match the title you see in the form builder, don’t worry—it’s not a bug.
In some cases, especially when using a template to create your form, the form response title may appear different. This is due to a setting called the Title Template, which allows you to dynamically generate response titles based on answers provided in the form.
The Title Template is a powerful feature that helps differentiate responses by including values from key fields (e.g. names, IDs, dates). For example, instead of a generic title like “New Response,” your form might show something like “Incident Report - John Smith - 14 May 2025.”
If your response titles aren’t what you expected, it’s likely that the Title Template was inherited from the original template and hasn’t been updated to match your form’s structure. To fix this, simply update the Title Template in the form settings to reflect the fields you want to include.
