Title templates are an advanced feature that allows form builders to control the text that is displayed in the responses tab. This text can be completely dynamic based on fields in the form response. This is also why the title in the response tab may be different to your form title. Follow the steps below to access this feature:
Navigate to the form builder screen for your form
In the top right-hand corner click the "..." icon to reveal a menu
Click "Title Template" within that menu and update the template.
To add field from within your form dynamically simply type the name of the field and you will see auto suggestions appear.
For example, in the demonstration below we dynamically change the tile of the response based on the response fields to help differentiate it from other responses to the same form. For complex fields like SharePoint Datasources or dates, your can click the dynamic fields for additional configuration.
