With the exception of the enterpise subscription, all Team Forms subscriptions are on a per team basis, allowing you to only pay for the teams which you need to host forms. For licensing purposes, a team is a Microsoft Teams group. For example, the screen shot below shows three seperate teams.
You do not need to purchase a subscription for each team, just the ones you want to host forms within.
